What you Need to Qualify for your Medical Marijuana Card:

*21 years of age

*Copy of Medical Records proving medical conditions and current with in the past 12 months

*Once Medical Records have been received a physician will review the records for eligibility of Medical Marijuana

*Certification Appointment- go over medical conditions and will sign a certification if you meet qualifications

*Have a current State Photo I.D.

Below is the Step-by Step Process for receiving your Medical Marijuana Card

Qualification for the Massachusetts Medical Marijuana Act requires a strict certification process. While we must adhere to Massachusetts law in our duties, we endeavor to make the medical marijuana application process as quick and easy as possible for our patients. Following are the steps in the qualification process.
First and foremost, you must have a medical condition that qualifies you for the use of medical marijuana under Massachusetts state laws. If you suffer from any of the conditions below, you could receive palliative benefits from the use of medical marijuana:

  • Agitation of Alzheimer’s Disease
  • Amyotrophic Lateral Sclerosis
  • Cancer
  • Crohn’s Disease
  • Glaucoma
  • Hepatitis C
  • HIV/AIDS
  • PTSD
  • Nail Patella
  • Cachexia or Wasting Syndrome
  • Severe Nausea
  • Severe and Chronic Pain
  • Seizures
  • Severe Nausea
  • Severe and Persistent Muscle Spasms

 

Step One: Medical Record Collection

The first step in the medical marijuana application process is extensive documentation of your medical condition. Without current and verifiable medical records, our physicians will be unable to certify your condition under Massachusetts law. Although there are conditions where a patient will qualify for the Massachusetts Medical Marihuana Program without the necessity of full documentation, the Boston Medical Marijuana Certification Clinic (BMMCC) requires that all your records are current within the last 12 months that document your qualifying condition.

Step Two: Physician Review

Upon receipt of your medical records, the physicians thoroughly review the records to determine if you qualify for the Massachusetts Medical Marijuana Program. If additional documents are required for the review, a  staff member should contact you and let you know which documents are needed for our records. If you qualify for the program,  call to schedule an appointment with one of the companies above.

Step 3: Certification Appointment

At your certification appointment, the doctor will talk with you about your qualification for certification of a medical condition that may qualify you for the Massachusetts Medical Marijuana Programs license, along with the physician visit fee., usually $200 depending on the company.s medical marijuana application fee.

Step 4: Application for Medical Marijuana Identification Card

As this time the requirements to receive your official Medical Marijuana Identification Card has not been completed by the State. We will update this requirement as soon as possible.

  • Signed and dated State Medical Marijuana Application Form
  • Physician’s Certification
  • Caregiver Attestation (if applicable)
  • Photo ID
